Your masseur will require you to complete this and retain it so that you can confirm the information on the day of your massage treatment during a pre-treatment discussion.

There are some important elements to this questionnaire and your honesty is appreciated as this will ensure that you get the best experience from your session.

Online: Submit the online "Massage Customer Information Form" (also downloading and using your device's drawing software to circle or mark areas that you want me to conentraten on before attaching it to an email).

Paper: Method 1: Download the paper form to print and complete.
